Summary Plan Information
Universal Life
• Choose a level of permanent coverage to meet your financial needs.
• Modified Guarantee issue (Two Medical Questions) up to $12 a week at Open Enrollment ONLY.
• Spouse & child(ren) coverage available.
• Long Term Care benefit included in this coverage.
• Portable.
Short Term Disability
• Choose from a 14 day or a 30 day elimination (waiting) period with a 6 month benefit period.
• Receive up to 60% of your gross monthly income to a maximum of $6,000 a month.
• Modified Guarantee issue $300 to $6,000 monthly up to 60% of your base income (2 Medical Questions) at Open Enrollment ONLY.
• Portable.
Accident
• Pays lump sum dollar amounts for ER visits, hospitalization, ambulance services, fractures, dislocations, and other accident related benefits.
• Plan reimburses $50 twice a year for individual coverage and four times a year for all other coverages for any doctor visit outside of a hospital,
including your family doctor, OBGYN, eye doctor and dentist for cleanings. • Employee, employee/spouse, employee/child(ren) and family coverage available.
• Semi-monthly cost is $7.76 for individual coverage
Cancer
• Plan pays lump sum benefits for specific treatments when diagnosed with cancer and twenty other specified diseases
• Pays $50 to $100 annually (depending on level of coverage chosen) for specific wellness tests such as Pap smear, colonoscopy, PSA, etc. and an additional $75 for mammograms.
• Semi-monthly premium is $11.24 for individual coverage.
• Individual and family coverage is available.
• Portable.
